本研究は，広島県の観光地である三段峡における車椅子走行時の走破性や介助者負担について，人力車アタッチメントの使用の有効性について検討することを目的とした。調査は「三段峡正面口～竜ノ口」までの約500m とし，通常歩行・人力車アタッチメント装着車椅子介助走行（以下，人力車W/C）で実際に移動を行い，移動時の負荷を評価した。評価項目は，血圧・脈拍・Borg スケール・介助負担感に関する質問紙調査を，走行前後に行った。その結果，通常歩行・人力車W/C の両方で，移動後の脈拍上昇，人力車W/C のみで移動後のBorg スケール上昇が認められた。群間の移動前後の変化量は有意な差を認めなかった。介助者が負担を大きく感じた部位として，「上腕」の回答が最も多く，困難に感じた経路は，主に「勾配」「道幅」「路面」の問題が挙がった。本研究の結果から，三段峡における車椅子ユーザーが観光を行う際，介助者における物理的および心理的負担が整理された。 |

The purpose of this study was to investigate the effectiveness of using a rickshaw attachment for wheelchair running and caregiver burden in the Sandan-kyo Gorge, a sightseeing attraction spot in the Hiroshima Prefecture. The study was conducted over a distance of approximately 500 m from the main entrance of to the Sandan-kyo Gorge to the Ryunokuchi entrance, where the subjects actually moved by normal walking or and wheelchair-assisted driving with a rickshaw attachment (hereafter, rickshaw W/C), and the load during the movement was evaluated. Questionnaires on blood pressure, pulse rate, Borg scale, and sense of burden were administered before and after the run. as a result, an increase in pulse rate was observed in both normal walking and rickshaw W/C, and an increase in Borg scale was observed in rickshaw W/C only. There were no significant differences in the changes observed before and after the movement within each group. The “upper arm” was the most frequent response as the part of the body where the caregiver felt a greater burden, and the problems of “slope,”, “road width,”, and “road surface” were mainly reported raised as the factors paths where the caregiver felt difficulty. The results of this study indicate that there agree physical and psychological burdens on caregivers when wheelchair users visit the Sandan-kyo Gorge. |
