通所リハビリテーションにおける認知症支援に関する課題を抽出することを目的として，H通所リハビリテーション施設（以下，H デイケア）を利用する認知症者を対象に，作業療法士の立場から事例を分析し，課題検討を行った。認知症のタイプ別の作業療法の実践として，H デイケアでの事例を５つ挙げ，評価，生活状況，サービス利用の経過，認知症者の変化を報告し，ケースを通じての課題を検討した。認知症者の病型内訳は，レビー小体型認知症１例，アルツハイマー型認知症３例であった。その他，１事例は H デイケア利用者の家族支援を行った経過を挙げ，生活状況，デイケアでの対応を報告し，家族支援の課題を検討した。これらの事例検討の結果，デイケアにおける認知症ケアの課題として，リハビリテーション専門職の役割，介護支援専門員との連携，協業，通所リハビリテーションにおける訪問の重要性について３点をまとめた。 |

We conducted a case study involving dementia patients using services provided by H ambulatory rehabilitation center from the standpoint of an occupational therapist in order to identify challenges in dementia support services as a part of ambulatory rehabilitation. We selected 5 users who actually received occupational therapy for different types of dementia at H center, assessed their conditions, daily life, use of services, and their changes, and investigated challenges based on these cases. The disease types included Lewy body dementia (1 person) and Alzheimer’s dementia (3).A case o which did the family support was indicated. Correspondence by day care to its life situation was reported and a problem of the family support was considered. This case study identified 3 challenges in dementia care provided as day care: roles of rehabilitation professionals, communication/working with a care manager, and the importance of home visits in ambulatory rehabilitation. |
